Completing the client registration form will keep you informed about Society activities, connect you with our Regional Service Coordinators and allow access to our services. Proof of diagnosis is required, for example a letter from your neurologist, but there is no cost to register and membership is not required. You may wish to obtain membership but this is at your discretion.
In order to receive any of the services in which we provide for people with MS, you must register with our services department. Please complete our client registration form (download here), attaching proof of diagnosis (a letter from your neurologist, for example) and send it to:
